Cancellation Policy
A handling fee of $125.00 will be deducted from each cancelled registration made between August 30, 2005 and September 16, 2005.

No refunds will be given for cancellations after September 16, 2005.

Cancellations for either the hotel or the Colloquium must be made directly with
The Eye-Bank for Sight Restoration, Inc. in writing or by phone at 212-742-9000.

The fees indicated above include a continental breakfast, lunch and one ticket to the cocktail reception on Friday September 30, 2005 as well as a continental breakfast on Saturday October 1, 2005.
